Dr. Hoshizaki's personalized concierge healthcare program provides participating patients with numerous benefits, including a greater emphasis on prevention and wellness.  At its heart, it affords each member one on one access to a physician who has devoted over 30 years to practicing and elevating the art of Internal Medicine, as evidenced by testimonials from patients and colleagues alike. By joining his concierge program, you will realize all the advantages of transitioning from a Primary Care Provider to a truly Personalized Care Provider. Whether you are advancing in age, healthy and interested in staying well, have a family history of a particular disease or have one or more chronic medical problems, you owe it to yourself to explore all that the program has to offer.

Program Benefits include, but are not limited to:

1. 24/7 direct physician access via telephone which will reduce urgent care and Emergency Room visits, as well as office visits.
2. Non-urgent confidential email communication with Dr. Hoshizaki via our secure patient portal.
3. Same  or next day office visits with  no waiting
4. An annual executive-level physical, which includes the following:
1. Complete medical history
2. Comprehensive  diagnostic and preventive care examination
3.  Korr ReeVue Metabolic Analysis to calculate your Basal Metabolic Rate (BMR) and strategy for weight loss
4. Body Composition scan to help determine dietary and exercise treatment goals.
5. Spirometry to assess lung function and respiratory status
6.  Personal Fitness Assessment to tailor an exercise program based upon your age, medical condition and fitness goals
7. Nutritional evaluation and plan
8. Personal review by Dr. Hoshizaki of all lab and test results
9. Web access to written documentation of key findings and all test result

Effective April 1, 2015, Dr. Hoshizaki will only be seeing patients who are members of his concierge program.  All other patients will be seen by one of the other highly qualified providers in our practice.
