Internal Medicine Associates Welcomes You To Your Patient Centered Medical Home!

The Patient Centered Medical Home (PCMH) is a comprehensive team approach to primary care, focused on making each patient an active participant in the healthcare decision process. In order to achieve recognition as a PCMH, the practice must complete a rigorous year long training program and obtain certification from the National Committee on Quality Assurance (www.ncqa.org). The PCMH renders care by following evidence based guidelines (i.e. national standards for prevention, evaluation and treatment) proven to result in superior outcomes and improved quality of care.


As a team-based approach to healthcare, the PCMH will:

  1. Provide easy access to our Providers, including same day appointments for both acute and routine care. We provide equal access for all patients.
  2. Provide 24/7 telephone coverage for problems arising after hours, with 24/7 access to all of your medical records in emergent situations.
  3. Implement the latest preventive health care recommendations, and follow current guidelines for managing both acute and chronic illness.
  4. Coordinate your visits with services provided outside of our office, including but not limited to specialty consultations, hospital admissions, home health care, behavior health referrals and public and private community services.
  5. Provide self-management instruction and tools with educational resources to help you understand and meet your health related goals.
  6. Provide confidential web portal access so that you have immediate access your personal medical documents and test results.
  7. Assist in the transfer of medical records both to and from our practice to maintain continuity of care. Our office contact for record requests is Dawn Cashman.
  8. Assist you in establishing Durable Health Care Power of Attorney and Advance Directives for end of life planning and care.


As an active participant in our healthcare team, we ask that you:

  1. Stay involved and present all medical questions and concerns to the healthcare team.
  2. Keep the team abreast of any OTC medications or supplements you are taking, report any issues of compliance in taking the medications we prescribe, and notify us if you are taking any medications prescribed by other providers.
  3. Utilize our 24/7 availability to discuss acute or urgent problems before going to a Treatment Center or Emergency Room.
  4. Make sure we receive records regarding any care rendered outside of the PCMH.
  5. Relay any concerns or suggestions you may have to improve our office workflow and level of patient care.
