New Patients

Welcome to Internal Medicine Associates, S.C.! In order to prepare for your first visit, please review the following guidelines:

  1. Our office runs very much on time. Please arrive 15 minutes before your scheduled appointment to allow for registration and form completion.

  2. We require all patients to bring their current insurance card to each visit.

  3. We require a minimum of 24 hours advance notice for appointment cancellations. A $30 fee is assessed for appointments canceled or missed without the requested advance notice.

  4. We strongly recommend that you check with your insurer regarding "Wellness" coverage. Please review our "Wellness vs. Diagnostic" explanation.

  5. For safety and health reasons, children are not allowed in the examination rooms. We suggest that you make alternate arrangements for child care prior to your visit.

  6. All co-pays are due at the time of service. We accept cash, personal checks, Visa, MasterCard, and Discover. We do not accept American Express.

  7. We ask that you print out and complete or review the following forms prior to your visit.

    1. Please print out and complete the Patient Registration Form.

    2. Please print out and complete the Patient Medical Questionnaire.

    3. Please print out and complete the HIPAA Communication Profile.

    5. Please print out and sign the HIPAA Consent Form.

    6. Please print out and sign the Advance Beneficiary Notice for Wellness Coverage.
